DISCOUNTED RURAL HEALTH VISITS!
May be available based on family size and income. To learn more about or enroll in our Sliding Fee Discount program, ask for the Patient Account Manager at any center. No one will be turned away due to their inability to pay. Completed sliding fee applications can be submitted at any center. You must show proof of income as part of the application process in order to determine if you are eligible for discounted services. Our Outreach department will first determine if you are eligible for Medicaid or other insurance options. Sliding Fee Application
