Chesapeake Bay Bridge Tunnel Toll Program
This program provides tickets to Eastern Shore residents traveling across the Bay for medical appointments based on income and family size. You don’t have to be a Rural Health patient to qualify.
Chronic Care Management
Chronic Care Management (CCM) is a Medicare sponsored program that allows us to more closely monitor your health and well being between office visits through monthly phone calls. Participants must be on Medicare and have two or more chronic conditions. Ask staff if you’re eligible to join.
Diabetes Prevention Program
The PreventT2 lifestyle change program is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). This program will help you make modest lifestyle changes and cut your risk of type 2 diabetes by more than half. The program is FREE to qualifying pre-diabetic Eastern Shore of Virginia residents. To see if you qualify for the program contact a Health Educator at one of our centers.
Foreign Travel Prep
We offer a foreign travel clinical service where prospective travelers can receive counseling on common illnesses and recommended immunizations.
Health Education
Health Education is a free service for all Rural Health patients. Health educators can help you manage your asthma, diabetes, blood pressure and cholesterol and they can advise you on weight loss, nutrition, exercise, and how to take your medicine. The health educators offer diabetes prevention and Healthy Habits for Children programs that include these services. They can also link you to help to stop smoking.
Help Getting Prescriptions
Can’t afford your medications? We can help!
Through our free Pharmacy Assistance Program, a medication assistance caseworker can review all of your medicines and determine the least costly way to get them for you.
Insurance Assistance
We have a team of certified application counselors who can help you understand your federal Health Insurance Marketplace and Virginia Medicaid and FAMIS programs for children, pregnant women and adults insurance options. Plus we help you complete enrollment applications!
Medicare Annual Wellness Visits
As a patient who has Medicare part B insurance you are allowed one Welcome to Medicare Wellness Visit at no co-pay within the first year of having Medicare part B insurance. Every year after the initial visit, Medicare covers the cost of a follow up Wellness Visit at no co- pay to discuss preventive care. You do not have to have received a Welcome to Medicare visit to be eligible for a follow-up Annual Medicare Wellness Visit.
Physicals
Sliding Fee Discount
Discounted visit may be available based on family size and income. Ask for the Patient Account Manager at any center. No one will be turned away due to their inability to pay. Ask the patient account manager at your center for a sliding fee application.
