WHAT: 6000+ fliers were just mailed to all ESRHS Medicare patients.
WHY: To notify Medicare patients that ESRHS is now part of an Accountable Care Organization (ACO). We are part of the Aledade ACO.
WHAT IS AN ACO: Doctors, hospitals, and other health care providers cooperating to give coordinated high quality care to Medicare patients. Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.
WHAT YOU NEED TO KNOW: The flier direct patients with questions to call the ESRHS main number, 757-414-0400.
- ESRHS Medicare patients are automatically in our ACO. NO ACTION IS REQUIRED!
- The portal the flier refers is our ESRHS portal to get test results etc., not a Medicare portal.
- When our ACO succeeds in both delivering high-quality care and spending health care dollars more wisely, it will share in the savings it achieves for the Medicare program with ESRHS. All involved benefit.
